Purpose: To report the first case of suprachoroidal triamcinolone injection used in treating nodular posterior scleritis.

Methods: Single-case, retrospective review of ophthalmological and systemic manifestations.

Results: A 67-year-old woman with nodular posterior scleritis diagnosed by spectral-domain optical coherence tomography scan had persistent pain, decreased vision, and redness in the left eye for the past 8 months. She had been prescribed topical prednisolone and prednisone with minimal improvement. She also had a history of rheumatoid arthritis treated with etanercept. She was given an injection of suprachoroidal triamcinolone in the left eye. Pain, visual acuity, and inflammation improved at the 11-day follow-up and 5-week follow-up.

Conclusions: Suprachoroidal triamcinolone may be investigated as an adjunctive therapy in refractory cases of nodular posterior scleritis.
