Analysis of factors associated with recurrence in Vogt-Koyanagi-Harada disease

Purpose: This study aimed to identify factors associated with recurrence in patients undergoing treatment for Vogt-Koyanagi-Harada (VKH) disease.

Methods: A retrospective review was conducted on patients treated for VKH with a minimum follow-up period of 6 months. Recurrent cases were identified based on the recurrence of inflammation, and comparative analyses were performed on clinical factors and treatment regimens between recurrent and non-recurrent cases.

Result: A total of 16 patients (30 eyes) were included. At onset, the average age was 50.3 ± 15.0 years, with 56.3% being female. Recurrence occurred in 7 patients (43.8%) at an average of 5.7 ± 4.0 months after the initial presentation. The recurrent group exhibited an older initial age (60.4 ± 7.6 vs. 42.4 ± 14.9 years, p=0.008) and a higher oral steroid dose at 6 months after presentation (14.3 ± 13.4 vs. 2.0 ± 3.5 mg, p=0.016). However, no significant differences were observed between the two groups regarding initial bilateral involvement, initial steroid pulse therapy, usage of immunomodulatory therapy at 6 months, > +2 cells at presentation, systemic symptoms at presentation, and initial visual acuity.

Conclusion: VKH patients with recurrent disease were initially older and had higher dose of oral steroids at 6 months. Predicting VKH recurrence based on clinical factors may be limited.
